INSURANCE
Participating Insurance Providers
We participate with the following
insurances. Please read the information provided by your insurance plan
to see if you need a referral before being seen and to make sure your
particular plan participates. If your insurance is not listed here
please see Non-Participating Insurance
for more information and options.
Medicare
United Healthcare
Blue Shield of NENY
Includes Traditional Blue, Community Blue and Senior Blue Plans
CDPHP
Includes all plans. If you have the General Electric Preferred Health Plan a referral is
required before being seen. For more information see our referral policy.
Empire Blue Cross Blue Shield
Please check your identification card, if your ID# begins with YLN, YLT, YLF, or YHA,
your plan will not cover services at our facility. Your visit will need
to be paid for in full at time of service.
Fidelis
We accept Fidelis, however we currently have one provider who is non-participating. Please call before you come in to see if
your insurance will consider your visit.
GHI
We accept all plans,
however, if you have GHI HMO a referral is required before being seen.

Medicaid
We do not accept Medicaid, however, we do accept managed care Medicaid such as Medicaid
offered by CDPHP, Wellcare, Fidelis, etc.
MVP
We accept all plans,
however, if you have the Albany Medical Center plan a referral is
required before being seen. Please see our referral policy.
Oxford Health Insurance
We participate with only one plan offered by Oxford. It is the Oxford Health Plan/United Healthcare Choice Plus Network. To see if you have this plan, on the back of your insurance card, in the bottom left corner it would say "United Healthcare Choice Plus Network".
Worker's Compensation
Please bring in the name of your company’s worker's compensation carrier and the address as well as a contact person's name and phone number at your place of employment.
No Fault
If your injury is at
all related to your automobile, please bring in your car insurance
information. See No-Fault Insurance information.
Non-Participating Insurance
If you have health insurance, but we do
not participate with your plan at this time, you are still welcome to
be seen. However, payment is expected at the time of service (please
see Payment Policies).
We will be happy to submit a courtesy claim to your insurance company
and you will be reimbursed according to your plan.
Cost of Visit: Our office visit fee averages $135.00. Any
additional services such as x-rays, sutures, labs etc, are billed in
addition to the office visit.
No Insurance
Our policy is that payment must by made at the time of visit. Acceptable methods of payment are:
- Cash
- Check (with license # and expiration date)
- Credit or DebitCard

Forgot Your Insurance Card?
Our policy is that payment must by made at the time of service. Acceptable methods of payment are cash, check
(with license number and expiration date) and credit or debit card.
Our office staff will attempt to verify a patient's insurance
eligibility by accessing the insurance company's website. If this is
not possible, we can hold the time slot for a patient while the patient
runs home to get their identification card.
If insurance eligibility can't be verified, or a patient is unable to
provide their insurance identification card, the patient must pay for
the visit in full at the time of service.
Our office visit fee averages $135.00. Any additional services such as
x-rays, sutures, labs etc, are billed in addition to the office visit.
If the patient does bring the insurance cards in to us at a later time,
we will make the appropriate adjustment.
Payment Policy
Payment is due at the time of service in
the form of cash, check, credit or debit card, or valid insurance card.
Copays are due at the time of service and are generally collected at
check-in.
Our office visit fee averages $135.00. Any additional services such as
x-rays, sutures, labs etc, are billed in addition to the office visit.
All past due patient balances must be paid in full before being seen.
Checks from patients in collection, past or present, are not accepted.
Cash or credit cards will be accepted.
No-Fault Insurance
Definition: NO FAULT law was enacted in
the 1970's and applies to anyone injured as a result of an automobile
accident or any action taken to get in or operate a car. This includes
the following:
- Fastening seatbelts
- Slamming fingers in car door
- Falling getting into or out of a car
- Loading a car or truck
The following information MUST be provided:
- Date of accident/injury
- The auto insurance carrier's name, address (not the Insurance Agent's info)
- The policy number
- A completed NYS No Fault Assignment of Benefits Form (provided by our office)
Referrals
Some insurance plans require a referral for services at our facility. If your plan is one of these (please refer to our insurance section) contact your
primary care physician before coming in to EmUrgentCare. If you are unable to provide a referral at the time of service, you can still be seen. However, you will need to sign an insurance waiver stating that you have three business days to get a referral to us. If we do not receive a referral within three business days, the cost of the visit becomes your responsibility.
