Payment is due at the time of service in the form of cash, check, credit or debit card, or valid insurance card. Copays are due at the time of service and are generally collected at check-in.

Our office visit fee averages $135.00. Any additional services such as x-rays, sutures, labs etc, are billed in addition to the office visit.

All past due patient balances must be paid in full before being seen.

Checks from patients in collection, past or present, are not accepted. Cash or credit cards will be accepted.
