Payment is due at the time of service in the form of a valid insurance card, cash, check, credit or debit card. Copays are due at the time of service and are generally collected at check-in. Please bring in the name of your company’s worker's compensation carrier and the address as well as a contact person's name and phone number at your place of employment.
